Formidable forms are not showing

Hi

Formidable forms are not showing up as a selection when I create my template do you perhaps know why?
It was there before. I am using version 1.11

Hi,

Do you see other available extensions in the list to choose?

- Recheck please if Formidable Forms plugin active and file exists under:
/wp-content/plugins/formidable/formidable.php

- Recheck please if file exists under:
/wp-content/plugins/e2pdf/classes/extension/e2pdf-formidable.php and not empty

Can you please turn on "Debug" mode via "E2Pdf" -> "Settings" -> "Turn on Debug mode", and check if it will appear, if still not, go please to "E2Pdf" -> "Debug" and check if you see "formidable/formidable.php" in "Plugins" section. Also, which browser are you using?

Also check please if you didn't disable extension by mistake under "E2Pdf" -> "Settings" -> "Extensions". It MUST NOT be set any checkboxes as on screenshot.
