Glitchy

When viewing the attachment in the email, the results from a form toggle to with check boxes on pdf can be glitchy (saying yes when the response was no, checking both yes and no).

I am wondering if how I have coded the outputs are taking up too much memory and is creating the glitches.

attached is an example with the exported health history pdf and a pdf of the email with the actual results.

Can you please attach screenshots of the "checkboxes" fields from the "Syndrome" for "Yes" and "No" via "Right Mouse Click" -> "Properties" inside E2Pdf Template, where we can see the "Option" and the "Value"?
