making sure form is appearing on site

I've had some work done to help with the form issue on the site. Can you please verify if you are seeing forms on the following pages?

The document testing page is where the forminator/e2pdf is setup. I'm not seeing it on my end but others are saying they're seeing it.

Unfortunately the forms are still not visible for me.

P.S. If you have any cache plugins you can try disable them, as in some cases it can cause when same page can work differently for different users.

I had wpmu dev hummingbird pro running nad I turned it off.

What is causing forminator not to work?

Yes, I can see that Formidable Form works fine. So it seems that Forminator plugin works incorrectly or it has conflict with your theme. It's better to contact Forminator team directly and figure out if they can assist you in solving your issue.
