Actions not working

Hi - I am trying to use the actions and they do not seem to work.

I have set the actions, and the fields still show even though they should be hidden. I have attached screenshots of the settings and entry. As you can see there is nothing showing in elective codes which is the [952] value, so the field should not show but it does.

Any ideas what I am doing wrong?

Your action will be applied only when it will meet "All" conditions:

If [925] like 335803 and [891] equals COMPETENT

If the setting is correct, create please "textarea" field and put there shortcodes [925] and [891] to see which output you get inside PDF.

We also noticed that you set action to "Show" instead of "Hide", however it must be used "Hide" action to hide fields/elements.
