I have used e2pdf with good results, but I am creating my first pdf with checkboxes but the pdf merge always shows it checked even if blank on the form. One other thing to mention is that in the form I have the checkbox status provide a numerical value to count the overall number in a survey and give a score. Any help is appreciated.

Hi,

Check please if you have correct "Option" and "Value" set on checkbox under field properties ("Right" mouse click -> "Properties").

"Checkbox" will be checked if "Value" has "Option" inside it. If you have value set to [123] Formidable Forms shortcode try to create "input" or "textarea" field with [123] as value to see which data you will get upon rendering.

You can also upload some screenshots so we can see and understand the reason why issue caused and if field can be setup in different way to have correct behavior.

thank you for the explanation. Now I'm closer.

1. In my example, I have a checkbox section with 5 options. When I map a field it always maps to the first option. that explains why yesterday I was getting all the checkboxes the same.

2. I cut and paste text in properties to put in the correct options. but these are not working. My delivered PDF only has a checkbox for the first option.

It looks like mistake in Formidable Forms Form checkbox setting. You are using same value for all options... It must be used different values for different options or you can remove "Use separate values" setting.

Try please to make corrections and remap checkbox(s).

I understand but, I'm using the "Use Separate Values" to create a numerical value. It is totaled, along with conditional styling to provide a ranking for the user.

So i'm guessing if I use this to return "1 point" for each checkbox to add to a score it isn't possible to also have it fill correctly to the pdf?

Unfortunately it's not possible to identify which value will be checked due the way Formidable Forms works with separate values. It will be always saved first option and same value as on attached screenshot.

The only way to make it work is to have Formidable Forms save it correctly and calculate ranking maybe by using some hooks / javascript (depending how you make calculation) as E2Pdf depending on entry data.
