Hello, my goal is the following:
1. recreate a not so perfect looking pdf to an online form
2. Have the Completed answers populate a new - better looking pdf
3. Have the pdf emailed to the end user and the website administrator of the site where they completed the form.

Details: Website is in DIVI - Prefer to make form in DIVI FORMS

Now, my main question is based upon the complexity of the original pdf questions, can this be accomplished in DIVI Forms?

I accomplished this goal before about 99% using Gravity Forms and Gravity PDF Extension, however the client wasn't happy with how the vanity aspect of how the online form and completed pdf didnt "lool like the original pdf"

What is the best plan of attack in accomplishing my goal?

E2Pdf is 3rd party software and we can't advice you which forms will fit your needs but DIVI FORMS can be good solution to create simple forms, but unfortunately functionality limited comparing to Gravity Forms / Formidable Forms. It doesn't store submissions like Gravity Forms / Formidable Forms, doesn't have Conditional Logic, it can be issues with sending multiple emails. So it can be problem with creating complex form like yours.
