There was no change in the site, and the settings seems to be all fine, but the pdf is not attached to the mails for some reason. I Will really appreciate your help. this is active site and is suppose to send cancel requests to insurance companies so it is quiet argent. Thanks.

Ok, I guess reactivating the plugin fixed it for some reason.
