hi there, when exporting formidabble forms and creating a new auto PDF all the form fields from here https://otahuhuhealthcentre.wpmudev.host/enrol/

are not showing at all on the form PDF , can see on the attachede checkbox

how can i make all form fields show on PDF

Hi,

From the image above we see that PDF showing fields similar to the form. Do you see "Auto" Generated PDF with only 1 page? If so, this is the limit of "free" version.

If you already applied paid License Key and still "Auto" Generated PDF showing only 1 page or some fields missed, please attach backup of the form so we can try to replicate issue on our side.
