I'm not able update my Template Title and PDF Name. Even if I delete the template and create a new one, it always create a new template using the deleted Template Title and PDF Name.

Never mind, I found the problem. I have the LastPass extension installed on my web browsers and for some reason it conflicts with your plugin. As soon as I disable the extension, the plugin saves correctly. You might want to look into resolving it.
