If "Caldera Forms" checkbox has option "Show Values" - %slug% shortcode will output "Values" instead of "Labels". To output Labels it will be need to use %slug:label% shortcode.

- You can enable true "Turn on Debug mode" at "E2Pdf" -> "Settings" and check if it will show any "red" errors on any tabs of "E2Pdf" -> "Debug"

However we believe that error is not common and can be connected with plugins conflict or some added filters/actions. If you still do not want allow us access to your live website, we can offer you to create duplicate website of current one and remove information that you do not want us to access, so we can login, see your form setup, make some tests and determine why action not fired via FTP.
