i'm getting Form could not be parsed due to incorrect html when i click the map field option. Attached is the pdf i'm using.

Is the issue with "Forminator"? Do you have any additional specific "HTML" in that form or just default fields? If it's possible can you "Export" form to ".txt", "zip" it and attach to next message please so we can try to replicate the issue on our side.

P.S. The problem not in PDF but in the "Form"
